Change, reissue, cancellation, and transfer of "Category Three Personnel" certificates

1. Application Materials:
(1) The company issues the "Three Categories Personnel" certificate change application form, loss replacement application form, or cancellation application form;
(2) Original and photocopy of the "Three Categories Personnel" certificate;
(3) Other supporting materials:
① For personnel transfer, provide proof of consent for transfer from both units;
② If the original certificate cannot be provided for cancellation, a unilateral statement of labor contract termination with the original unit or the labor contract with the new unit must be provided;
③ For certificate loss replacement, provide the original and photocopy of the newspaper publishing the loss declaration.
